AACOM/AOA Annual Osteopathic Medical School Questionnaire

Welcome to the AACOM and AOA joint survey of osteopathic medical schools.

Deadline for Submission: January 23, 2006

The information sought in this questionnaire is collected and used for information and research purposes, and will be provided to the AOA as part of the institutional accreditation process. It will be used as follows:

  • In preparing AACOM's "Annual Report on Osteopathic Medical Education."
  • To furnish the American Osteopathic Association and AACOM with identical basic reference data.
  • For special studies being conducted by the AOA and AACOM.

Information provided in this questionnaire will be released with institutional identifications in accordance with the following policy:

  1. UNRESTRICTED DATA (marked "u"): may be made available to the general public on an individual school basis.
  2. RESTRICTED DATA (marked "r"): may be made available only in aggregate (system-wide) terms; not available outside of AACOM/AOA on an individual school basis unless authorized by AACOM's Board of Governors.
  3. CONFIDENTIAL DATA (marked "c"): held strictly confidential; will be released only with the permission of the Dean or President of the submitting institution.
The data classification (u, r, or c) is appended to each question.

Answer all questions fully. Please do not leave any items blank.

Some questions and/or sections must balance like any other accounting sheet. Make sure that all entries add correctly going across and/or down to the total.

Please pay careful attention to the years of reference in each question since some items are requested for the prior year. Include in "Fourth Year" column or row those in their fourth, fifth, or sixth year and not part of the graduating class. This may differ from the number of students you enter in the "Graduates" column or row.
